Becoming A bella Butterfly

Thank you for your submission to the Arabella Models Affiliate Program. I, ___________________________________________, hereby authorize Arabella Models Image Consulting (Dear Arabella and Alt-Bella Magazine),  to use my photographs, reproduce, and/or publish photographs and/or video that may pertain to me or photographed by me, or my event including my images, likeness and/or voice with payment of a monthly commission through the Arabella Model Affiliate Program.  This allows each one of our talents that will be modeling in our Showcase the Arts Fashion and Beauty Expo and for Bella Butterfly Fashion to be involved in our profit sharing programs.  Each Affiliate Model that is featured on our runway at the Spring or Fall Expo will be given a unique promo code. This will give your fans $1 off their ticket to the expo. For each affiliate you will  will need to sell 2 tickets to qualify for a pay out. Affiliates will receive 50% of all tickets sold with your unique promo code.

Promo codes will also be active on the Bella Butterfly Fashion Website, each Affiliate Model will receive 10% of every sale made with their promo code.  Affiliate Models will receive 40% off all items the model would like to purchase for themselves.  All Social Media Influencers (That meet requirements) and Models (Who have over a $600 in sales each month) will receive a clothing allowance for Bella Butterfly Fashion as well as a monthly pass into an Arabella Shootout (Shootout open for AZ Affiliates only at this time).
I understand that my image may be used in either Dear Arabella or Alt- Bella Magazine and may also be posted along with your credit on the Alt-Bella or Dear Arabella Magazine facebook, youtube, IG or www.arabellamodels.com.  
This authorization is continuous and may only be withdrawn by my specific rescission of this authorization. Consequently, the Corporation or project sponsor may publish materials, use my name, photograph, and/or make reference to me in any manner that the Corporation or project sponsor deems appropriate in order to promote/publicize Showcase the Arts Fashion and Beauty Expo, Bella Butterfly Fashion, Dear Arabella Magazine or Alt- Bella Magazine.

You will be notified if your submission is accepted 48 hours after your submission.
